The "Skulled" Cap is a throwback to the years of playing OSRS.  
 
Being "Skulled" referred to a status that would be applied to you after you attacked another player. Often referred to as PK'ing or Player Killing.  When "Skulled" a large Skull would appear over your head. This Skull would signify that if you were killed, you would lose all your items. By having this skull you were  feared by some, or a prime target for others. 
 
For Law Enforcement, the Military and any responsible armed Citizen who conceal carries on a daily basis you know that when you leave your home for the day,  you and your families lives, your belongings, and potentially your freedom is on the line.
